What happens to my items when I die with Requiem?

Instead of losing them immediately, your character transforms into a soul or undead form. What happens to your items depends on which form you choose and your mod settings. You get more control over the death mechanic than vanilla Minecraft provides.

Can I switch back to vanilla death after choosing Requiem?

The mod only prompts you once when you first die. After that, you'll need a special item to change your death system. Choose carefully on that first death.

What undead forms can I become after dying?

You can become a soul, zombie, drowned creature, revenant, or phantom. Each form has different mechanics. Zombies burn in sunlight, drowned creatures breathe underwater, and others have unique abilities.
